Community Care Behavioral Health, York/Adams Behavioral Health Managed Care Program, has started a new maternal program.
- This community-based care management program is designed to enhance support for pregnant women and new mothers.
- A care manager will be available to follow-up with pregnant members/new mothers to assess their behavioral health needs and basic needs and link them to behavioral health services, community resources, and other supports as needed. They will make regular outreach to members, up to one year post-partum, to continue to provide support.
- The only criteria for participation is that the woman be a Medical Assistance recipient, and therefore CCBH eligible, and either pregnant or within 1 year post-partum. If you know she would qualify for MA, but is not yet enrolled, we can assist with the MA application.
- To make a referral or for more information, call 888-251-2224. When making a referral, we just need the member’s name, DOB, MAID (if known), contact info (phone number, address), pre/post natal status, and reason for referral.
